What a Medicare Advantage plan actually is
Medicare Advantage is the privatised version of Medicare: instead of the government paying providers directly, an insurer takes a fixed payment per enrollee and manages that person's care, usually bundling in extras that traditional Medicare does not cover. The economics reward insurers who can add benefits cheaply — which is precisely where a warehouse retailer has something unusual to contribute.
The plans are designed to plug into things Costco already sells: prescription drugs, over-the-counter products, vision care and hearing. Costco runs one of the larger pharmacy and optical operations in American retail, and those are exactly the benefit categories Advantage plans compete on.
The restriction that shapes the whole idea
Federal rules prohibit bundling a Costco membership with the health plans. The single asset Costco would most want to attach — the membership that renews at a famously high rate and defines the entire business model — is the one thing regulation keeps out of the package.
That constraint explains why this is being run as a pilot rather than a national launch. Costco has to establish that its value proposition survives without the membership hook: that people choose a Costco-branded Advantage plan because the pharmacy and optical benefits are genuinely better, not because it comes attached to a warehouse card.
Why the share move is modest
SCAN Health Plan covers about 460,000 members across 33 counties in 6 states, and the wider SCAN Group network reaches roughly 500,000 members and patients. Those are real numbers, but next to Costco's global membership base they are small, and the products still need regulatory approval before anyone can enrol.
So the near-term earnings effect is close to nothing, which is what a 0.03% move reflects. The reason to pay attention is strategic rather than financial: a retailer with a pharmacy footprint entering managed care is the same convergence that reshaped the pharmacy sector when CVS bought Aetna. If the pilot works, the interesting question is not what it adds to this year's revenue but whether Costco's cost discipline transfers into an industry with very different margins.
