How much of the beat came from reserves

UnitedHealth's medical care ratio, the share of premiums paid out in medical costs, fell to 88.4% from 89.9% a year earlier. The release says the ratio was helped by $610 million of favorable prior-period reserve development for 2026 dates of service, which is money the company had set aside earlier in the year for claims that turned out to cost less. Mizuho calculated that the ratio beat expectations by about 150 basis points and by about 80 basis points without the reserve benefit, so close to half of the margin beat came from releasing money set aside in earlier quarters rather than from the third quarter's own claims.

The second quarter had leaned on the same source, with $860 million of favorable development, so reserve releases have now supported two consecutive beats.

Q2 202686.7%$860 million$6.38
Q3 202688.4%$610 million$4.69

The ratio usually climbs through the year as members meet their deductibles and the insurer pays a larger share of each claim, which is why it rose from 83.9% in the first quarter. The new floor of $20.50, set against $18.30 of adjusted earnings in the first three quarters, leaves about $2.20 a share for the fourth quarter.

Why the opening gain did not hold

Chief financial officer Wayne DeVeydt said 2027 earnings growth would come in at the low end of the company's 13% to 16% long-term target. Coverage that morning also pointed to enrollment figures that came in slightly below expectations and to the reliance on reserve releases. Revenue of $112.7 billion beat estimates but was below the $113.2 billion of a year earlier, partly because revenue at Optum Health, the care-delivery business, fell 10% as it served fewer patients under value-based contracts.

The selling did not carry into the next session. On Wednesday, October 14, UnitedHealth rose 4.73% to $395.62 while the Nasdaq fell 1.88%, without a new company announcement.

Why did UnitedHealth stock fall after beating earnings on October 13, 2026?

What was UnitedHealth's medical care ratio in Q3 2026?

88.4%, against 89.9% a year earlier. It included $610 million of favorable reserve development for claims from earlier in 2026.
