Sight Sciences (SGHT) shares jumped 18.544% in the minute after the company announced FDA 510(k) clearance of its OMNI Ultra Surgical System on August 4, 2026, moving from $5.77 to $6.84 in the after-hours session, on thin volume. By the next regular session the stock was changing hands around $5.32, down 3.797% on the day — 22.2% below that after-hours peak, and 7.8% below where it traded before the clearance was even announced.

The clearance itself is real and specific. The FDA cleared OMNI Ultra for canaloplasty followed by trabeculotomy — a procedure that widens the eye's natural drainage channel and then creates small openings in it — to reduce intraocular pressure in adult patients with primary open-angle glaucoma, the most common form of the disease. OMNI Ultra is the next generation of the company's existing OMNI platform, which has been used in more than 350,000 procedures worldwide as of June 30, 2026. The company says OMNI Ultra is the first FDA-cleared minimally invasive glaucoma surgery device to perform circumferential canaloplasty — treating the full ring of the drainage channel — in a single pass, adding a technology called TruSync Plus for finer control during the procedure. Paul Badawi, the company's co-founder and CEO, called the clearance "an important milestone reflecting our continued investment and meaningful innovation" in implant-free glaucoma surgery.

What a regulatory clearance does not do is generate revenue on its own. Sight Sciences says the commercial launch of OMNI Ultra remains on track for the fourth quarter of 2026 — months after the clearance itself. A clearance announcement moves a stock on the promise of a better product; the actual sales, insurance reimbursement, and surgeon adoption that would justify a sustained re-rating still lie ahead, and none of that was disclosed alongside this release.

Two things compounded the reversal. First, the initial reaction happened after hours on light volume — a handful of trades can move a thinly traded small-cap stock sharply in either direction, and that kind of move is mechanically more fragile than one built on a full day's liquidity. Second, a separate market data analysis published alongside the news noted that company insiders had been net sellers of the stock over the trailing 90 days — a data point unrelated to the clearance itself, but one that tempers the case for reading the announcement as an unambiguous turning point for the shares.

None of this means the clearance is not meaningful for the business. It is a genuine expansion of what the company's flagship platform can do, and OMNI Ultra's single-pass, implant-free approach is a real technical advance over the original OMNI system. What it shows is a common pattern for small-cap medical device stocks: a real, positive regulatory event can produce a sharp after-hours pop that has nothing to do with near-term earnings, and that pop can fully unwind once the broader market has a full session to price in that the commercial impact is still a quarter or more away.
