Intuitive Surgical stock fell 10.57% within a minute of its third-quarter results reaching the MoveSurge news feed at 16:06:05 ET on Tuesday, October 20, 2026, dropping to $398.88 from $446.00. Revenue rose 22% to $3.05 billion and adjusted earnings per share (EPS) rose to $3.00 from $2.40, but U.S. da Vinci procedures grew only 10%, and management said its pricing changes will lower instrument revenue per procedure by about 3% to 5%.

Procedure growth drives most of Intuitive's revenue. Every da Vinci operation uses instruments and accessories that hospitals buy again for the next one, and that line brought in $1.80 billion of the quarter's $3.05 billion. Slower U.S. surgery growth and lower prices on those instruments both reduce it, which is why investors looked past the earnings beat.

Where procedure growth slowed

Worldwide procedures on da Vinci and Ion systems grew about 15% from a year earlier. Da Vinci procedures grew about 14% and Ion, the company's lung biopsy system, about 31%. The split by region carried the bad news: on the call, management said U.S. da Vinci procedures grew 10% while procedures outside the United States grew 20%. Call coverage described the U.S. figure as the slowest since the pandemic, partly because the third quarter of 2025 was a strong quarter to compare against.

Intuitive narrowed its 2026 forecast for worldwide da Vinci procedure growth to about 14% to 15%, from 13.5% to 15.5%, and said it expects to land near the middle. The midpoint stayed at 14.5%, so the narrower range dropped the top of the old forecast without raising the expected result.

The instrument price cut

Instruments and accessories revenue per da Vinci procedure was about $1,860, up from $1,800 a year earlier, helped by more single-port procedures and instruments made for da Vinci 5. Management said new pricing actions, together with a government-driven price reduction in China, would lower that figure by about 3% to 5% on a mix-constant basis once fully in place. Chief executive Dave Rosa presented the cuts as a deliberate choice to win more high-volume operations for benign conditions, where a lower cost per procedure helps hospitals move cases to the robot. On the July call, management had declined to put a number on the pricing change and promised more detail this quarter, so October was the first time investors saw its size.

A $450 million settlement and a tariff refund

GAAP EPS fell to $1.49 from $1.95 because of a $450 million pre-tax charge to settle antitrust class action litigation. The settlement, agreed in October without any admission of wrongdoing, cost $0.99 a share after tax and still needs court approval. Adjusted EPS of $3.00 excludes that charge but includes a $0.06 benefit from refunds of tariffs paid in earlier periods. Intuitive also bought back 7.8 million shares for $2.94 billion during the quarter, and its cash and investments fell by $1.83 billion to $6.80 billion.
